I understand I am renting the above kit for a fee of $40.00. I agree to arrange for pickup and return of the kit to Southeast Vermont Learning Collaborative (SVCLC) by the date specified below, or pay a $10.00/week late charge.  It is my responsibility to account for all materials in the kit (see attached inventory). Should any non-consumable materials or equipment become damaged or lost, I will reimburse SVCLC for the cost of replacement. Consumable materials are included in the cost of the rental.
